Receiving a dementia diagnosis is not the end of your journey; it is the beginning of a new chapter that requires ongoing adaptation and compassionate guidance. As circumstances shift and symptoms evolve, your care plan needs to evolve with them. For clients who have already completed their initial assessment and diagnosis with Peggy Rice, NP, or have a pre-existing dementia diagnosis from another healthcare provider, our dedicated follow-up care provides a safe, consistent space to anchor your progress. We are here to ensure that your treatment plan remains effective, supportive, and aligned with your family’s current reality.

Treatment Plan Review

In these ongoing sessions, we meticulously review the foundational treatment plan established during your initial assessment. We discuss what is working well, identify any new challenges, and adjust clinical recommendations to ensure you or your loved one continues to receive the highest standard of care.

Assessing Progress & Symptoms

Dementia affects everyone differently, and tracking cognitive and behavioral changes over time is vital. We use these 60-minute touchpoints to gently assess current cognitive function, mood, and daily living capabilities, providing a clear picture of how things are progressing.

Collaborative Problem Solving

Follow-up care is highly collaborative. Whether you are dealing with new communication hurdles, safety concerns in the home, or emotional distress, these sessions offer a space to troubleshoot in real-time and develop practical, immediate solutions.
