Our comprehensive dementia assessment is designed for individuals seeking clarity and a defined path forward without a prior diagnosis. This structured, empathetic evaluation helps identify cognitive changes and establishes a personalized treatment plan, ensuring you and your family feel supported and informed every step of the way.
Cost: $1800 | Clinicians: Occupational Therapist Julie & Nurse Practitioner Peggy Rice | Duration: 120-minute (or 2x 60-minute) session with Julie & a 60-minute & a 90-minute session with Peggy
Geriatric Assessment
The process begins with a detailed 120-minute consultation with a geriatric assessor to gather essential background information and medical history. Conducted by phone or virtually with a family member, this step ensures we understand the full context of your experiences before direct testing begins.
Cognitive Testing
Next, a 60-minute specialized cognitive evaluation is conducted. This session can take place in person or virtually, depending on your comfort and accessibility. We utilize structured testing to gently but thoroughly assess memory, focus, and overall cognitive function.
Diagnosis & Treatment Plan
The final step is a 90-minute collaborative family meeting (in person or virtual) to discuss the findings and outline a clear, actionable treatment plan. You will leave with a comprehensive strategy that can be managed through ongoing follow-up sessions with our clinic, or shared seamlessly with your Primary Care Provider.
